Applicant must submit their current valid, or recently expired, passport (and/or passport card if applicable) in order to renew. The old Passport will be returned marked “Cancelled” by the US State Department
Applicant must submit one completed Passport Renewal Application Form (DS-82) which is “barcoded” – handwritten applications are not accepted for expedited services.
Applicant must submit two identical, 2″x2″, color, passport-style photographs.
Applicant must submit an original or certified copy of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned.

If you got married recently and your passport is less than one year old, you can get a free name change using form DS-5504. If your passport is more than one year old you will need to apply for a new passport using form DS-82 if it has been less than 5 years, or form DS-11 if it has been more than 5 years. Generally no. Once you submit your old passport with your name change application you will not have it back until the new one arrives. If you have upcoming travel, tell us before you submit. If your passport is less than one year old the DS-5504 name correction is free. If you need a full new passport the government fee is $130 plus a $35 execution fee where applicable.
