US Passport Name Change Requirements

Prerequisites for Passport Name Change

(if the below criteria are not met, please select a different passport service above)
  • Applicant has US passport is in his or her possession, AND;
  • Applicant has legally changed their name since most recent passport was issued AND;
  • Applicant’s passport was issued within the past 15 years AND;
  • Applicant applicant was 16 years or older when passport was issued.

Note: We provide our customers complete assistance with your online application, Photos, and other supporting documents with Walk-in from 9 a.m. – 2:00 pm (Monday to Friday) Sat (9 a.m. till 1:00 p.m.) and Sunday (12pm to 3pm )or call us at 972-231-1600.

1 Current Passport / Card

Applicant must submit their current valid, or recently expired, passport (and/or passport card if applicable) in order to renew. The old Passport will be returned marked “Cancelled” by the US State Department

2. Passport Application (DS-82)

Applicant must submit one completed Passport Renewal Application Form (DS-82) which is “barcoded” – handwritten applications are not accepted for expedited services.

3. Passport Photos – (This service is available at our all locations)

Applicant must submit two identical, 2″x2″, color, passport-style photographs.

4. Name Change Documents

Applicant must submit an original or certified copy of the marriage certificate (not a license) or a court decree ordering a name change or an adoption. The document must show the official seal of the issuing authority. All official documents will be returned.

6. Letter of Authorization – Please call us at : 972-231-1600

Please complete and print out two copies of Authorization Letter. This letter will authorize our company to submit and obtain the new passport on your behalf. Please call us for a Letter of Authorization due to limited Slots available for passports Submission or walk in our office from 9am -3pm

7. Processing Time & Fees

A. U.S. Government Fee:

  • A personal check or money order made payable to the “US Department of State” in the amount of $190.00
  • If you are applying for a US passport with regular services, Check or Money order made payable to : US Department of State $130.00
  • Temporary/starter checks or checks without a name or address will NOT be accepted.
  • AA Passport and Visa Services fees will be applied in addition to the government fees.

8. Order Form

Click the button to completely fill-out order form. You will be asked to input your personal information, order information, and payment information. After inputting your Data, click on “Submit Order” which will create your completed order form, then print it out and sign it.
