Expedited Child’s Passport Instructions

Under Age 16 Child Passport Or Renewal Requirement
  • One parent appears, signs, and submits second parent’s notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, authorizing passport issuance for the child or submits a notarized, written statement of consent from the non-appearing parent (including the child’s name and date of birth, as well as parent’s identification information or a copy of his/her ID) authorizing passport issuance for the child.
  • Both parents must appears at the Passport Acceptance Facility to sign the application for the minor.

CHILD NEW PASSPORT OR RENEWAL

1. Completed Application DS-11 form online (No handwritten form )

2. Proof of U.S. Citizenship (one of the following documents)

  • A certified U.S. birth certificate
  • An expired U.S. passport
  • Original naturalization certificate

3. Two identical passport photographs :

Note: Passport photo services available at our offices.
Taken within the past 6 months, showing current appearance. Must be official passport-type only and approximately 2″ x 2″ with a clear background.

4. Proof of Identification

  • Parents State issued driver’s license (at least 6 months old) bearing a photo
  • Parents Military I. D. bearing a photo
  • Parents valid US Passport or Non US Passport

5. Proof of Departure

  • A copy of your itinerary from the travel agent or airline or
  • If you do not have a ticket or itinerary please Call at our office number at :972-231-1600

6. Letter of Authorization – Please call us at : 972-231-1600

Please complete and print out two copies of Authorization Letter. This letter will authorize our company to submit and obtain the new passport on your behalf.

7. Processing Time & Fees

A. U.S. Government Fee:

  • A personal check or money order made payable to the “US Department of State” in the amount of $190.00
  • If you are applying for a US passport with regular services, Check or Money order made payable to : US Department of State $130.00
  • Temporary/starter checks or checks without a name or address will NOT be accepted.
  • AA Passport and Visa Services fees will be applied in addition to the government fees.

8. Order Form

Must Appear in Person

Please keep in mind that you must come to our office in person to apply for a New Passport, Lost Passport, or Child Passport Processing. We will complete all necessary paperwork, such as an online passport application, photos, and a letter of authorization, and then you will be sent to a Passport Acceptance facility by appointment to have your document verified by a County Clark officer, who will seal the package and return it to you, which must be dropped off at Our Office to begin the process. This will be a hand-carried application to the State Department for a US passport.

To apply for a child’s passport under age 16, both parents or legal guardians must appear in person together with the child.
