- Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page) – photocopy is not acceptable
- One Visa Application Form completed and signed. The application form must have original signature.
- Two Passport Type Photographs
- Photocopies of the passport pages containing the full name, photo, passport #, issue date, and expiry date (one copy of each page)
- Computer generated flight itinerary from the travel agent
- For minors, a Parental Consent Form* from both parents allowing their dependents to travel to Lebanon is required

No. Lebanon does not admit travelers who have evidence of travel to Israel in their passport. This includes Israeli entry or exit stamps and Israeli visa stickers. If you plan to visit both Israel and Lebanon on separate trips you need a passport without Israeli entry evidence for the Lebanon portion.
