- Valid and signed U. S. Passport (valid for at least 18 months with at least one blank visa page) – photocopy is not acceptable
- One (1) Visa Application Form completed and signed. The application form must have original signature and MUST BE NOTARIZED in the USA – effective April 3, 2015.
- One (1) Passport type photograph (MUST be recently taken – within the last 3 months) – must be good quality with white background (no eyeglasses is allowed)
- Letter from the Employer stating that the applicant is employed and will travel to Afghanistan as a tourist.
- Computer generated flight itinerary

May 28, 2015, the validity of the visa is at the discretion of the embassy. The visa could be issued single entry for 3 months OR multiple entries for 1 year. If you want a 1 year multiple entries, the company must be on the list of the Embassy of Afghanistan as under contract with the US Government – this list is provided by their Ministry of Foreign Affairs. If you want to be included in the “list”, you have to contact the Ministry of Foreign Affairs in Afghanistan.
