US passport is required. Visa is NOT required for tourist or business purposes if the duration of stay is 30 days or less. Visa is required for over 30 days stay.
Visa Requirements for U.S. Citizens
CREW/SEAMAN 9(C) VISA
- Actual passport of applicant (valid at least 6 months at the time of entry) and one (1) photocopy of the data page.
- One Notarized accomplished visa application completed and signed. The application form must have original signature and must be notarized.
- One (1) colored photo, 2” x 2”, taken within six months before the date of application, showing a clear front view of applicant’s face, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted.
- Proof of ticket reservation. Foreign nationals MUST NOT finalize travel itineraries until after receipt of visas. The Embassy will not be liable for any missed flights due to proximity of the intended travel date.
- Letter from Shipping Company
- Proof of Financial Support from the applicant (current bank statement).
- Crew List form as prescribed by the Embassy
- Acceptable proof of vaccination (against COVID-19) i.e. CDC vaccination record card, state issued vaccine records.
- Actual Seaman’s Book
TOURIST VISA
- Valid and signed U. S. Passport (valid at least 6 months beyond the date of entry; with at least one blank visa page) – photocopy is not acceptable
- One Visa Application Form must be completed online. Important: On the ” Country of Application” section, please choose “United States of America” and on the section “Embassy/Consulate” drop down menu, please choose “Washington DC Philippine Embassy”
- One (1) colored photo, 2” x 2”, taken within six months before the date of application, showing a clear front view of applicant’s face, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted.
- Computer generated flight itinerary from the travel agent or copy of roundtrip airline ticket
- Notarized parental consent from the non-traveling parent(s) – applicable for applivants under 18 years old.
- Proof of Financial Support from the applicant (current bank statement).
- NOTE: For minor applicants (under 18 years old), the parents must write a notarized affidavit of support for the minor and also copy of their current bank statement.
BUSINESS VISA
